2014 Michigan Work-Related Deaths

Case Summaries

The table below contains preliminary information about Michigan workers who died as a result of an injury sustained while at work. The information has been obtained from public media sources, MIOSHA, and stakeholder reporters. MIFACE has not verified this information; verification may conclude that the death is out of scope. 

A MIOSHA program-related fatality is determined by whether the individual who died was employed in an industry included in MIOSHA jursidiction as defined in Public Act 154 of 1974, as amended and the fatality may be related to a MIOSHA standard. 

Case #12 and Case #21 have been determined to be out-of-scope. Cases #139, #145 and #149 have been determined to be non-work-related.

 5.  1/16/14 Dishwasher A dishwasher at a senior center died as a result of  medical complications due to a fractured ankle sustained in the parking lot of his employer. As he was walking, he slipped on the ice. Several weeks later, while in the hospital, he died from medical complications of the injury. Jackson

 18.  4/2/14 Truck Driver A truck driver died when the ethanol-containing tanker truck he was driving left the roadway, and struck a tree causing the contents of the tanker to ignite/explode. Tuscola

 20.  3/14/14 Library Assistant A library assistant was leaving work at the end of the day when she apparently fell on cracked concrete in front of the library. She was transported to the hospital and had surgery to splint the injury sustaned as a result of the fall. While in the hospital, medical complications occurred, which caused her death. Lapeer

 23. & 24. 4/19/14 Pilot & Business Owner A pilot and his passenger (a business owner) died when their plane crashed while trying to make an emergency landing. A witness to the crash indicated that the plane had its landing gear down. The pilot was taking the passenger to another city to pick up a helicopter that had been serviced. The passenger used the helicopter for both business and pleasure purposes. Shiawassee

 28. 4/30/14 Truck Driver A truck driver died when the semi he was driving struck an overpass support, causing the semi to be sent sideways across the traffic lanes. The semi was struck by an oncoming semi. The collision caused both semi tractors to burst into flames. The decedent was unable to escape the burning semi tractor. Monroe

31. 5/21/14 Truck Driver A semi truck driver died when the semi he was driving left the road and rolled into a ditch. The semi trailer was carrying boxed milk. Newspaper reports indicate that the driver was not wearing a seat belt and was ejected from the cab during the rollover. Barry
32. 5/27/14 Truck Driver A semi truck driver died when the semi he was driving left the roadway, traveled through a ditch and hit several trees. There had been a crash that had occurred in front of him and he swerved off the roadway to avoid striking other vehiches that were slowing down due to the crash. The tractor cab caught fire and the driver was pinned in the cab by the semi's engine which had been pushed back into the cab as a result of the collision iwth the trees. Calhoun

36. 6/26/14 Truck Driver A truck driver was shot while waiting to pick up a load from a facility. It appears that the decedent was first shot and then the perpetrators set his semi cab afire. Wayne

41. 6/29/14 Truck Driver A truck driver was walking near the fog line on an expressway when he was struck and killed by a vehicle whose driver left the scene. Newspaper reports indicate he was in the process of checking his non-functional trailer tail lights. It is unknown if he was walking to the back of the truck or if he had checked the lights and was returning to his cab when the hit-and-run occurred. Oakland
42. 7/10/14 Bank Courier A bank courier died when the vehicle he was driving was struck head-on. The decedent was traveling eastbound on an expressway, following a vehicle towing a boat. The vehicle towing the boat saw a vehicle traveling westbound in the eastbound lanes and swerved to get out of its way. The decedent could not react quickly enough to avoid the oncoming vehicle.    Ottawa

45. 7/29/14 Owner A tree trimming business owner died when the dump box of a dump truck fell onto him. Newspaper reports indicate he was working on the truck's hydraulics at the time of the incident. Macomb
46. 7/28/14 Grocery Store Worker A grocery store worker died due to gunshot wounds during an attempt to stop a vehicle hijacking. Wayne
47. 6/9/14 Farmer A farmer was struck by a 35-foot tall tree with a 12-15 inch diameter he had just felled. The tree had been hanging over a farm field. The decedent did not notch the tree prior to making his felling cut. The police report indicated that the tree had been cut and shifted, striking the decedent and landing on top of him. Gratiot

54. & 55. 9/9/14 Case Workers Two case workers died in an motor vehicle accident. According to newspaper accounts, the two individuals were in their vehicle waiting to make a left turn when their vehicle was rear-ended. The collision caused their vehicle to enter oncoming traffic. Jackson
56. 9/17/14 Church Volunteer A volunteer for a church-sponsored food bank/community center died when she was struck by a backing vehicle. Eaton
57. 9/26/14 Truck Driver A waste hauling truck driver died as a result of a collision with a pedestrian bridge. According to newspaper accounts, the boom of his truck was fully extended when it struck the pedestrian bridge. Wayne
58. 9/17/14 Truck Driver A truck driver died when, for unknown reasons, lost control of his semi and the semi rolled over into a ditch. Wayne
59. 6/19/14 Custodian A custodian was struck by a bus as the bus was making a left turn at an intersection with a flashing red light. The decedent was walking across the street using the pedestrian crosswalk.    Washtenaw
60. 10/9/14 Logger A logger was pinned under a tree he had felled. The decedent was cutting two separate trees. Tree #1 had been felled, but became lodged in Tree #2 as it fell. The decedent used a second chainsaw and made a cut on Tree #2. The chainsaw was wedged into Tree #2. He then returned to make a relief cut on Tree #1 with another chain saw when Tree #2 splintered and broke. A part of Tree #2 fell onto him. Charlevoix

73. 7/15/14 Construction Worker A construction worker, who was a passenger in a pickup truck, died in a motor vehicle accident. The decedent was enroute with his coworkers to a jobsite when the pickup truck struck a bridge span loaded on a semi-trailer. The bridge span, which had no lighting, reflectors or flagging in the middle, was positioned across both lanes of the two-lane roadway when the pick up truck drove under and collided with it, causing the pickup truck's roof to be crushed downward. 74. 5/30/14 Business Owner The business owner was working with a family member to dislodge a bulldozer. The family member backed a pickup truck to the bulldozer. The decedent and family member attached a tow strap to the bulldozer and under the ball of a hitch mounted to the pickup truck. While the decedent "gunned" the bulldozer in reverse, the family member "gunned" the pickup truck forward. The hitch sheared, and the sheared ball/hitch struck the decedent. Hillsdale
75. 6/24/14 Bus Driver A bus driver died when his bus crossed the centerline, across oncoming traffic lanes, and then struck a building. The driver's side of the bus impacted the building causing the decedent's fatal injuries. Wayne
76. 8/11/14 Pilot/Business Owner A business owner was piloting an Evolution Trikes Revo weight-shift-control aircraft which sustained an in-flight breakup. A witness stated she "heard an engine revving up", followed by a "loud bang". Walking outside, she observed an aircraft wing descend from the clouds. The aircraft crashed, killing the pilot. Allegan
77. 3/14/14 Custodian Supervisor The decedent fell on ice in a parking lot while taking trash out to a dumpster. He died several months later from complications of his injury. Grand Traverse
78. 3/26/14 Self-employed The decedent was on a ladder working on an overhead bay door in a service garage when the ladder slipped out from under him. He fell approximately 12-15 feet to the garage floor. Macomb
79. 2/14/14 Construction Laborer A construction laborer fell from a two-story roof while clearing snow. He died several weeks later from complications of injuries sustained at the time of the fall. Wayne
80. 3/6/14 Business Owner The business owner fell inside his shop and broke his leg. He died from post-surgical medical complications. Oakland
81. 6/7/14 Horse Trainer A horse trainer was training his draft horses when one of the horses broke free. The decedent pursued the horse. The decedent and horse continued into a field and then into a wooded area. The decedent was found dead in the wooded area. It appeared to responders that he had been dragged and that he had struck a tree. Branch
82. 1/31/14 Hotel Co-owner The decedent was working with two other co-workers tarping a stack of carpeting located outside of the hotel. One coworker was using a forklift to elevate the decedent and his coworker approximately 8 feet from the floor to place plywood on the tarp so it would not blow away. The decedent and his coworker were standing within a large wooden box/crate held aloft by the forks of the forklift. The box/crate was not secured to the forklift. The decedent's coworker walked to the side of the box where the decedent was standing, causing it to become unbalanced and fall from the forks to the floor. The incident occurred in 2006 and the decedent succombed to injury complications in 2014. Oakland

85. 7/14/14 Vehicle Driver A vehicle driver for an auto auction company died as a result of a motor vehicle collision. The decedent was driving a vehicle from one auction company to another auto auction company. The decedent was traveling eastbound and crossed the centerline. The vehicle struck an oncoming westbound vehicle. The crash report indicated that the westbound vehicle disregarded traffic control. Cass

87. 11/11/14 Delivery Driver A delivery driver died when his vehicle struck a train. The decedent was approaching a train crossing and applied his brakes. Due to the icy road conditions, the vehicle did not stop in time. His vehicle struck the train. The decedent was ejected from the vehicle. He died several days later as a result of the injuries sustained at the time of the incident. Menominee

93. 5/1/2014 Business Owner A business owner died in a motor vehicle crash. The decedent, who was driving a pickup truck was traveling southbound when he failed to yield at an intersection to an oncoming semi truck. The decedent was not wearing a seatbelt/shoulder harness. Cass
94. 7/30/14 Landlord A landlord was electrocuted when he contacted a fallen, energized, 30,000-volt power line. The energy company had placed yellow caution tape around the area of the downed power line. The decedent climbed over a fence and entered the cordoned off area to clear debris from the yard when he inadvertently came into contact with the power line. Macomb

96. 12/7/14 County Sheriff A county sheriff died in the line of duty as a result of a vehicle crash. The decedent was pursuing a vehicle at a high rate of speed when his patrol car left the roadway and struck a tree. Ingham
97. & 137. 2/15/14 Laborer A farmer (driver) and a laborer (passenger) died when the pickup truck  was struck by a train on the passenger side. The incident occurred on private property. Genesee
98. & 99. 8/23/14 Social Organization Volunteers Two volunteers died when the float in which they were passengers struck a vehicle at an intersection. The decedents were unrestrained passengers seated in the back of the float. The float had the right of way. A vehicle turning left entered the roadway from the intersection and was broadsided (driver's side) by the float. Lapeer

101. 9/11/14 Construction Volunteer A carpenter who was volunteering in a garage building project died while trying to fix the garage's roof. He fell approximately 10-20 feet from a ladder to the concrete below. Marquette
102. 12/24/14 Company Owner A company owner died when the twin-engine plane he was piloting missed an approach to a runway, hit some trees, and crashed.   St. Clair

109. 3/20/14 Truck Driver A truck driver died when the semi he was driving left the roadway and overturned. Calhoun
110. 1/30/14 Laborer A laborer died from a self-inflicted gunshot wound. Berrien
111. 5/9/14 Laborer A laborer died when he fell from a barn roof. The laborer and other workers were replacing a ridge vent at the roof peak. Huron
112. 12/11/14 Store Owner A store owner died from a gunshot wound. Wayne
113. 2014 Furniture Mover A furniture mover died from a drug overdose. Wayne
114. 7/20/12 Manufacturing Inspector A manufacturing inspector fell at work in 2012 and died from injury complications in 2014. Ingham
115. 12/30/14 Logger A logger died when a tree he was cutting fell onto him. Branch

117. 3/12/14 Regional Manager A regional manager for a fast food company died in a motor vehicle crash. Vehicle 1 was traveling northbound on a divided highway in the left lane. Vehicle 1 lost control, crossed the median and struck the decedent's southbound vehicle. Jackson
118. 8/21/14 Farmer A farmer died when he was struck on the head by 5-foot wide hay tongs. A team of horses bolted which caused a block and tackle weighing about 40-50 pounds to be ripped off its track from the top of the barn to the floor. The hay tongs were attached to its own block and tackle, and struck the decedent as they were projected past him. Mecosta

120. 11/1/14 Carpenter A carpenter was helping to install an overhead commercial garage door while standing near the top of a step ladder. A spring loaded tool snapped, striking the decedent's head. He lost consciousness on the ladder, and then fell to the ground. Genesee

122. 6/26/07 Brickmason A brickmason died in 2014 from complications of a head injury sustained in a 2007 construction incident. The decedent was working on the ground floor erecting a block wall when a 12-foot-long metal wall stud fell from the second floor, striking his head. Macomb
123. 2/6/79 National Guard A member of the National Guard was injured in a motor vehicle accident. The decedent was driving and lost control of the vehicle. The vehicle rolled over and he sustained neck injures. He died of complications of the injuries in 2014. Eaton
124. 11/19/13 Auto Repair Shop Owner An auto repair shop owner died as a result of an explosion. The decedent was using a worn and damaged web trpe strap to remove a vehicle fuel tank. The strap broke causing fuel to be released and ignited by a propane heater used to heat the garage. Ingham

126. 9/29/14 Salesman/Farm Worker A salesman/farm worker died from a self-inflicted gunshot wound. Berrien
127. 11/19/14 Party Store Owner A party store owner died due to a fall from a ladder. Genessee
128. 8/12/14 Farmer A dairy farmer died when a freestanding grain bin crushed him. It appears that his horses and/or manure spreader struck the bin as they were running away and, while he was running after the horses, the bin fell onto him. Clare
129. 12/23/14 Farmer A farmer was driving an all-terrain vehicle to a cornfield when he lost control of the ATV. The incident occurred in 2012, and he succombed to complications of the injury in 2014. Sanilac
130. 9/22/14 Excavating Company Owner An excavating company owner died from a self-inflicted hanging. Mackinac
131. 4/21/14 Automotive repair An automotive repair business owner was found overcome by gasoline fumes in a 7-foot pit in a pole barn. He had been working on a car that had been leaking gasoline. The decedent had positioned the car directly over the pit.  Tuscola
132. 9/2/2014 Farmer A farmer died from a self-inflicted hanging. St. Clair
133. 7/2/2014 Farmer A farmer drowned when the lawn mower used to cut pathways out to his fields overturned into a ditch filled with approximately one foot of water. The lawn mower appeared to have been traveling in reverse when it overturned into the ditch, pinning the decedent. The mower did not have a roll over bar. Monroe
134. 8/3/2014 Home improvement A home improvement laborer died when a garage wall collapsed onto him while he was attempting to lift the garage. Macomb
135. 11/29/14 Human Resources A human resources director died from a self-inflicted gunshot wound. Barry
136. 10/2/14 Farmer A farmer died while using an old tractor with a blade on the back to cut grass. The decedent turned the tractor to the right while in a ditch. The front left tire of the tractor went up and over a culvert, causing the tractor to tip up and the decedent to fall off the seat backwards, landing on the ground in back of the blade. The tractor then rolled back, trapping the decednt partially under the blade mechanism. Saginaw
138. 5/13/14 Construction Laborer A construction laborer died in a motor vehicle crash. The decedent was driving a pickup truck on a we roadway with a posted speed limit of 45 mph. The decedent was driving in the left lane and, according to witnesses, had previously crossed the center line marker. The decedent's pickup left the roadway to the right, striking a cub, then entered a ditch. The pickup struck a fixed decorative pole and then went airborn as it cleard a private driveway. The decedent's pickup then struck another vehicle on the side of the roadway. Kent.
139. 7/16/14 Farmer A farmer died due to complications sustained in a tractor incident. It is unclear whether the decedent died due to being pinned by the tractor against a wall or whether the was run over by the tractor. St. Clair
140. 1/24/13 Process Designer A process designer was driving a passenger vehicle when his vehicle struck a parked tow truck in 2013.  The decedent was southbound on an icy 4-lane roadway with a posted speed limit of 70 mph. He was traveling in an outside lane which looked clear, but had a thin layer of ice. The decedent lost control of the car and it rotated counter clockwise approximately 110 degrees and then slid into the rear of a tow truck that was parked and working to remove another vehicle that was stuck in the snow. The decedent was wearing the seatbelt. The  airbag did not deploy. Allegan
141. 1/31/14 Farmer A farmer died due to head injury complications sustained when he backed over a wood pile with a covered ATV. The ATV overturned to the rear. The decedent struck the top of his head on the roof when the ATV flipped. Gratiot
